We have some important information to share with you regarding your appointment:
1. Please look for an email from us with forms to fill out prior to your new patient appointment. If you are applying insurance, it is important we have this information two days prior to your visit. You can put it on your emailed forms or call us with this info.
2. We ask that you arrive 10 minutes early for your appointment to complete some additional new patient forms.
3. All patient copays are due at the time of service. Please arrive prepared with cash, check, debit or credit card to pay any applicable copays due at your appointment. Thank you!
